What we collect
If you submit a form on this site, we collect only what you type in: your name, date of birth, phone number, email address, and preferred contact method (appointment form), or your first name, last initial, city, and story (patient story form). We deliberately ask you not to include medical information in these forms — your health history is discussed privately when we talk.
How we use it
Appointment requests go to our scheduling team, who use your information for one purpose: contacting you to schedule a visit. Date of birth helps the team find or create the correct patient record. Patient stories are reviewed by our team and published only with the permission you grant on the form.
What we never do
We do not sell your information. We do not share it with anyone outside the practice and the secure form service that delivers it to us. We do not send marketing email lists.
Analytics and cookies
Like most websites, we use Google Analytics and Google Ads measurement to understand how visitors find and use the site — pages viewed, buttons clicked, and whether a visit came from an ad. This measurement is anonymous: the contents of forms you submit are never sent to analytics. Videos are embedded through YouTube’s privacy-enhanced player, which does not set tracking cookies unless you play a video.
